Chronic Health Conditions, Concussion and Screenings

When students enroll and at the beginning of every school year parents are asked to update their student’s health history. Our goal is to make sure that health care concerns do not become a barrier to learning. After receiving additional information about the chronic health condition from the student’s health care provider and/or parent, a health plan will be developed as needed and shared with appropriate staff. The school nurse may provide care directly to the student or train other school staff to give medications or assist with specialized health care procedures.

Diabetes

  • Cape Fear Center for Inquiry will comply with NC law Diabetes Bill SB911.
  • Students with diabetes must have a Diabetic Medical Management Plan (DMMP) or similar document completed and signed by the student’s healthcare provider.
  • DMMP or similar document must be signed by the student’s parent/guardian.
  • A Physician's Authorization for Medication at School form must be completed for each medication required.
  • School staff assigned to care for the student with diabetes will meet with the student's parent/guardian before the first day of school to review the DMMP and care provided by the school.
  • All diabetic supplies will be supplied by the student’s parent/guardian.
  • CFCI cannot provide any diabetes care to students without the required completed paperwork from the child's health care provider or necessary supplies and medication.
  • If a student is allowed to carry and self-administer medication, a back up medication must be kept at the school per G.S.115c-375.2.

Asthma

  • Students with asthma must have an Asthma Emergency Action Plan (AEAP) completed and signed by the student’s healthcare provider.
  • AEAP must be signed by the student’s parent/guardian.
  • A Physician's Authorization for Medication at School form must be completed for each medication required.
  • School staff assigned to care for the student with asthma will meet (in person or via phone) with the student's parent/guardian to review the EAAP
  • All medications (including rescue inhalers) will be provided by the student’s parent/guardian.
  • If a student is allowed to carry and self-administer medication, a back up medication must be kept at the school per G.S.115c-375.2.

Allergy and Anaphylaxis

  • Students with a severe allergy or anaphylaxis that requires medication if exposed must have an Emergency Action Plan (AA-EAP) completed and signed by the student’s healthcare provider.
  • AA-EAP must be signed by the student’s parent/guardian.
  • A Physician's Authorization for Medication at School form must be completed for each medication required.
  • School staff assigned to care for the student with a severe allergy or anaphylaxis that requires medication if exposed (in person or via phone) with the student's parent/guardian to review the AA-EAP
  • All medications (including epinephrine auto-injectors) will be provided by the student’s parent/guardian.
  • If a student is allowed to carry and self-administer medication, a back up medication must be kept at the school per G.S.115c-375.2.
  • CFCI will maintain a supply of emergency epinephrine auto-injectors in accordance with G.S. 115C-375.2A to provide emergency medical aid to persons suffering from an anaphylactic reaction during the school day and at school-sponsored events on school property.
  • A supply of emergency epinephrine auto-injectors provided in accordance with G.S. 115C-375.2A shall not be used as the sole medication supply for students known to have a medical condition requiring the availability or use of an epinephrine auto-injector.

Seizures

  • Students with seizures must have a Seizure Emergency Action Plan (SEAP) completed and signed by the student’s healthcare provider.
  • SEAP must be signed by the student’s parent/guardian.
  • A Physician's Authorization for Medication at School form must be completed for each medication required.
  • School staff assigned to care for the student with seizures will meet (in person or via phone) with the student's parent/guardian to review the SEAP
  • All medications (including rescue mediations) will be provided by the student’s parent/guardian.
  • If a student is allowed to carry and self-administer medication, a back up medication must be kept at the school per G.S.115c-375.2.

Concussion

A multidisciplinary team approach is taken in response to concussion related issues at school. The school can be notified of a suspected concussion by any of the following persons: healthcare provider, parent, teacher, school staff or athletic coach. Health Screening

Screenings are provided for certain grade levels. Students at any grade with evidence of problems related to vision or hearing will be screened or re-screened upon request. Parents/guardians of students will be notified of the date and time of screenings.

Nursing services include, but are not limited to:

  • Individual health care needs assessments for students
  • Health care plan development, revision and evaluation including Emergency Action Plans, Individual Health Care Plans and health-related components of Individual Education Programs
  • Direct provision of nursing health care services
  • Emergency and injury care, planning and training
  • School staff instruction and supervision in provision of care for students with special health care needs
  • Instruction and oversight for medication administration processes in schools
  • Communicable disease control and outbreak response in schools
  • Assessment of school community health and safety issues
  • Student screening for conditions that impact access to education, such as vision screening
  • Member and resource for student assistance teams that develop health-related student accommodations
  • Health-related community resource liaison for schools
